Mount Holly Partners with Samaritan to Launch Comprehensive Chronic Illness Support Model

Mount Holly Rehabilitation & Healthcare Center in Lumberton, N.J., has launched a comprehensive Chronic Illness Support program in partnership with Samaritan Palliative Medical Partners. This initiative integrates palliative care and pastoral support into specialized clinical programming for chronically ill patients. Administered by a trained medical team, the palliative care component aims to enhance patients’ quality of life and reduce hospitalizations. The program supports Mount Holly’s Pulmonary Care programming, including disease-specific education, coordinated care, and social services support. Mount Holly recently earned accreditation from The Joint Commission and recognition as one of the nation’s “Best Nursing Homes 2022-23” by U.S. News & World Report, while also undergoing renovations and programming enhancements.
